An agreement, Waiver, and Release for Minor

I am aware that participation in the Premier Basketball Training Unlimited program has some inherent risks and injury can occur. I now authorize the Premier Basketball Training Unlimited Program directors to act for me according to their best judgment in any emergency requiring medical attention to my son/daughter. I waive and release the Premier Basketball Training Unlimited Program, its coaches, volunteers, and any claims for personal injury. I will be responsible for any medical or other charges concerning their involvement in the program. I, with this, expressly permit said minor child to travel by private automobile to activities and events related to the basketball program. I now give consent to allow photographs of said minor. I understand Premier Basketball Training Unlimited may use the pictures. I attest that I am eighteen years or older, my child is physically fit, and I have no known medical conditions prohibiting participation in this sport. I have read this waiver and agee to the contents.
